REGISTRATION FORMS

The following forms are required and must be submitted at least 14 days prior to the first day of the selected TERP Quest week.

Sometime after submitting your TERP Quest registration, you will receive an email notice from the camp director informing you that the forms are available for download and reminding you of the requirement to complete and submit them.

• Release and Parental Informed Consent Statement
• Immunization Form

Failure to complete and submit forms by the due date will result in cancellation of registration without refund.
